What is it?Describe the item or memory in your own words.
Where did it come from?Name its maker, keeper, speaker, or previous custodian if known.
Who may see it?State whether it may be public, excerpted, restricted, or private.
Who can verify it?Suggest a family, lineage, or community reviewer where appropriate.
